All wrestlers must have all paperwork and tests completed before the start of the season. Any wrestler who is missing any forms will NOT be allowed to step on the mat, and will be forced to sit out until those forms are completed.
All wrestlers must have these completed before they are allowed to wrestle:
An up to date physical submitted to the school (ask us if you aren't sure if yours is expired, or will be expiring during the season)
A concussion test done every two years by our trainer
Forms completed online by a parent/guardian (all available on on website)
Permission Form
SCA Form
Concussion Form
Out of season Form
HIPPA form
Physical appointments should be make well before the season starts. We have had wrestlers in the past who waited to make an appointment and were forced to sit out the beginning of the season until they were able to be seen by a doctor. Your physical form must be submitted to the school, so for those who have had physicals recently - all you have to do is get a copy of it and bring it in to be eligible.
Both the coaches and trainer have a list of which athletes physical is OK and whose is expired/expiring soon. Ask us if you are unsure about the status of yours.
Every two years the trainer needs to do a concussion baseline test with you. The purpose of doing this test is to test your reaction time, memory capacity, speed of mental processing, and executive functioning and have this data available should you wind up sustaining a head injury in the future. more info can be found here.
Mark cannot test you after working out, so make an appointment to see him during your study hall of possible. If that is not an option, then see if you can have him test you at 3:00 before doing any sort of conditioning.
All forms need to be completed by your parent/guardian before you are allowed to take part in wrestling. There are 5 forms that you must fill out, and all are done through the sportsware website. www.swol123.net
The school ID is: Easties
The process does not take very long. Coach Tolla and I made a quick video that takes you through the process, so watch it if you have any questions. The video is linked below. Also - below I video I attached Sportsware's instructions if you have any other questions.
When you initially sign up, you must choose "new to Sportsware" and when it asks for a school ID type: easties
After submitting, it will go to Mark Aceto (the trainer) for approval, and you will be sent a temporary password which you can use to access your account (you can change password at this point if you would like to).
