Travel Release Form

Travel Release Form is linked below. In short:

• Athletes are ALWAYS to ride with the team to and from events regardless of age. 

• Exceptions may be granted in certain instances by the AD or a school administrator, and the coach has been made aware.

• Failure to follow the transportation rules (e.g., students driving themselves or being driven by unauthorized persons) will result in the student not being allowed to compete. 

• Students will only be released to their parent/guardian unless prior approval is granted for another arrangement. The parent/guardian must be present before the team's departure. 

• Submission of the form is required at least one school day prior to the event, and submitting it releases the school and its employees from liability regarding transportation deviations.
